Applicant Instructions
Part A - Instructions
Contact the Board office at 410-402-8560 to request an application.
- Complete the licensure application in full. Your photograph must be
attached with a Notary Seal impressed over a portion of the
photograph.
- The application fee is $200. The remittance address is Maryland
Board of Occupational Therapy Practice, Spring Grove Hospital Center, 55
Wade Avenue, Baltimore, MD 21228.
- Two moral character forms are to be completed and returned directly to the
Board (not returned to the applicant) .
- Verification of licensure forms must be completed by the Board(s) of
Occupational therapy from all state(s) or country(ies) in which you have
been licensed.
- Verification of Certification Request Form from the National Board of
Certification for Occupational therapy (NBCOT).
- Request for confirmation of examination registration and eligibility to
examine notice.
- Log your continuing education contact hours an submit with your
application, if applicable. Per COMAR 10.46.04, please include
documentation from the date you sign the application.
- Successful complete the jurisprudence
examination. If you cannot access
the exam by clicking on the link, visit the NBCOT website at www.nbcot.org
and select Regulators, and then Maryland Jurisprudence Examination
- For
Maryland Occupational Therapy License Applicants Only.
Part B - Licensure candidates previously NBCOT certified
All certified applicants must submit items 2, 3, 4, 5, 7 and 8 from Part A and the
following:
- Two self-addressed mailing labels
- Personal check or money order, in the amount of $200, made payable
to: MBOT, Spring Grove Hospital Center, 55 Wade Avenue, Baltimore, MD
21228.
Part C - Licensure Candidates Pending NBCOT Certification
All examination candidates must submit items 2, 3, 4, 6, 7 and 8 from Part A and
the following:
- Two self-addressed mailing labels.
- Personal check or money order, in the amount of $200, made payable
to: MBOT, Spring Grove Hospital Center, 55 Wade Avenue, Baltimore, MD
21228.
- Upon completion of the Certification Examination, the examination results
must be sent directly from the testing agency to the Board office.
(These arrangements may be made when registering for the NBCOT examination.)
Part D - Applicant Information
Once all information is received in the Board office, processing of the
application for official licensure averages 30 days. It is the
responsibility of the applicant to monitor the status of the application.
Upon successful completion of the certification exam and receipt of the
results by the MD Board of OT, a temporary license is valid until the official
license is issued.
Upon unsuccessful completion of the certification exam, a second temporary
license may be issued. The applicant may contact the Board office for
assistance.
