Official License

Official licenses are available for Occupational Therapists and Occupational Therapy Assistants.  All forms can be obtained by visiting the Forms page.


Official License Process

The process for official license is as follows:

A.    Request an application.  

B.    Complete the application, including:

  •     2 Self-addressed labels

  •     A photograph affixed.

  •     Notary Seal impressed over a portion of the photograph.

  •     Application fee ($200) money order or check payable to MBOT

  •     Successful completion of the jurisprudence examination.  If you cannot access
        the exam by clicking on the link, visit the NBCOT website at www.nbcot.org
        and select Regulators, and then Maryland Jurisprudence Examination - For
        Maryland Occupational Therapy License Applicants Only.  

  •     Continuing education documentation if it has been more than one year since you
        graduated from an O.T. academic program:

     

    •  24 contact hours if more than 2 years since graduation; or

    • 12 contact hours if more than 1 year since graduation but less than 2 years, or 

    • 0 contact hours if less than 1 year since graduation.

C.    Forward request to third parties responsible for sending material to the Board Office on your behalf. For example:

  • Letters of Verification from any state or country in which you've been licensed.  These are available in Word 97 format and Adobe Acrobat version.  

  • Attestations to moral character completed on forms provided by the Board Office.  Two (2) moral character
    forms are needed.  These are available in Word 97 format and Adobe Acrobat version.

  • Verification of initial certification or examination from the National Board for Certification in Occupational Therapy (NBCOT).

D.    Board Office forwards all applications completed by the 3rd Wednesday to the Board on the 3rd Friday of the month.

  • Applications approved for licensure are processed by the Board Office during the
    4th week of the month.  An official license will be sent in the mail.  License numbers are not given via telephone or fax.

  • Applicants not approved for licensure are notified.

Applications must be complete by the 3rd Wednesday of the month in time for Board Review on the 3rd Friday.
